You will soon be receiving a letter telling you how the NHS is developing the way your health information is stored and managed within Hertfordshire. During 2010 the NHS Summary Care Record (SCR) is being introduced to help deliver better, safer care.

You do not have to have a Summary Care Record and can choose not to have one by completing an 'opt out' form and handing it in to your GP practice. The form is available from your usual practice, or it can be downloaded here.
